How do I get a medical report? What type of report is required?

Even if you have attended several different doctors PIAB only require a single medical report to be submitted as part of your application. Any doctor that has treated you in relation to your injuries should be in a position to complete a report on your behalf. Please note your treating doctor does not need to carry out a physical examination for the purposes of submitting a medical report. It is recommended they complete the PIAB medical form based on your medical records in their possession.

What do I need to do if I am having difficulty getting a medical report?

You may submit your claim application form to PIAB in the absence of a medical report. You or your doctor may post it to us at a later stage. If you are experiencing difficulty getting a medical report, we recommend you contact your local GP to establish whether he/she could give assistance and provide us with a note outlining the nature and extent of your injuries. Alternatively you may provide a copy of your hospital admission records for our consideration. It is important to remember in most instances you have 2 years to make your claim. It is recommended you submit a completed application form, medical report and application fee within this time period.

Who pays for my medical report?

The cost of the report to be submitted with your application is a matter between you and your doctor. The reasonable cost of the completion of the report from your medical records will form part of your final award.
